What do I need for the appointment?

Your ID must be current or issued within the last five years. The name on your ID must match how you will sign.

Be sure to bring a proper form of ID* such as the following:

• State-issued driver's license

• State-issued identification card

• U.S. passport

• U.S. military ID

• State, county, and local government IDs

• Resident alien identification card (green card)

• Passports issued by other countries

• Federal identification cards

• Tribal driver's license or ID card
